Measure blood pressure, pulse rate, respiratory rate, and body temperature. Perform a "head-to-toe" physical examination, inspecting and palpating the patient's body from head to toe.
  • Medication Management and Administration: Managing medications effectively ensuring that patients understand their medication regimens, adhere to prescribed dosages and schedules, and are aware of potential side effects or interactions. Skilled nurses may also administer medications, including intravenous therapies, injections, and infusion treatments, under the direction of a physician, assist with medication planners, work with physcians and pharmacies to order medications and obtain prescriptions for refills.
  • Wound Care and Management: Assess, treat, and manage a variety of wounds, including surgical incisions, pressure ulcers, diabetic wounds, and traumatic injuries. Utilize evidence-based wound care techniques, such as wound irrigation, dressing changes, and infection control measures, to promote healing and prevent complications. Deliver specialized wound care in the home setting, and help patients achieve optimal outcomes and minimize the risk of infection or recurrence.
  • Chronic Disease Management: Many patients receiving home health care have chronic conditions such as diabetes, heart disease, or respiratory disorders that require ongoing management and monitoring. Skilled nurses provide education, support, and clinical interventions to help patients manage their chronic conditions effectively, prevent exacerbations or complications, and optimize their quality of life. Through regular assessments, symptom management, and lifestyle modifications, skilled nurses empower patients to take an active role in managing their health and well-being.
  • Patient and Family Education: serve as educators and advocates for patients and their families, providing valuable information and guidance on the disease processes, treatment options, self-care strategies, and community resources. Equipping the patient and caregivers with the knowledge and skills they need to navigate their health care journey, skilled nurses empower them to make informed decisions, adhere to treatment plans, and achieve their health goals.
  • Coordination of Care and Collaboration: Work collaboratively with other members of the interdisciplinary care team, including physicians, therapists, and home health aides, to ensure coordinated and comprehensive care for patients.
